Please prepare for your visit by bringing:

1. Health Insurance Card/Information
2. A list of all medications you are taking
3. A copy of your XRAYS/CT scans
4. Copies of records  from doctors related to your visit
5. Bring your sleep study and audiogram when appropriate
6. A list of your questions/concerns
